Case Management

Case Management assists you in developing a plan and then works with you to help obtain the supports necessary to make it work. The case manager assists with service coordination, problem solving and assuring that all supports are leading to an achievement of your plan and personal goals.

Home Based Services:

These services and supports are provided in your home and can include the obtaining of adaptive aides, nursing services, minor physical modifications to your home, occupational therapy, physical therapy, social services, speech and language services, behavioral support services, and supported home living (which includes both personal care and assistance in learning new skills).

Supervised Living:

This service supports you in your own house or apartment and provides staff to assist with you with personal care, learning new skills, or assisting with completing household chores such as cleaning, shopping and paying bills. The amount of support you receive is based on your needs and the plan you develop but does not provide staff in your home 24 hours a day.

Supported Family Homes:

These are homes where you live with a family or a companion that does not have a disability. This service is often referred to as “companion care” or “foster care”. This kind of living situation can be right for you if you wish to live with others and have them provide assistance to you. This assistance can be in the form of providing personal care, attendant care, skills training, room and board, assistance with medical appointments and any other needs you may have. Homes receive certification from AHS and we monitor them on a regular basis. In this option you choose the home and the people you would like to live with.

Respite:

This support will provide care and assistance when your family or usual support system needs to go somewhere without you. This can be provided in your usual home or in an alternative setting.

Residential Support:

If you need to have staff in your home 24 hours a day, this is a service alternative you may be interested in. This service is provided in a home you choose and typically you live in with one or two other people. The in-house staff provide all types of support that you may need and this support is designed by the service plan you design yourself.

Day Habilitation:

This is a day time activity that focuses on both skills development and providing enjoyable activities in your community. This service is for you if you wish to continue to learn new skills or want to enjoy being out in your community. Actual classes and activities vary depending on the people who attend the programs as well as what the group wants and needs to learn. Day habilitation can also be right for you if you wish to improve your skills as a way of gaining employment. In many places we offer volunteer opportunities as well as job skills training.

Employment Support:

This includes a variety of supports that are intended to help you become employed or increase your income. Employment support helps you determine what kind of work you may want to do and helps you in all aspects of procuring that work. It also includes supported employment where you work in a job and an on-the-site job coach assists you in becoming successful.

ICF/MR Facilities:

These are facilities which are certified under Federal and State stadards and provide services to individuals with intellectual and other qualifying disabilities. This is the only federally mandated service offered in Texas. In general, people who reside in an ICF/MR receive training, medical support, physical, speech or occupational therapy, dietary assistance and other supports. Because this is a mandated service, the number of facilities in the state is limited. There is no waiting list for these services in Texas at the current time and AHS now has openings in both small group homes and larger facilities. We make every attempt to make these settings person-centered by giving people living here choices about your staff and living environment. Please see the individual programs for a description of each facility. All other services are funded by the State of Texas and Federal Medicaid program as alternatives to ICF/MR programs.
